Azova Health At-Home Testing Collection Kit Coupon Code Request

Students who submitted financial aid applications and demonstrated significant financial need have been emailed a coupon code to purchase an at-home testing collection kit.   If you did not receive a coupon code and are unable to pay for your at-home testing  kit due to a financial hardship, please complete the Azova Health Coupon Code Request Form to explain your situation.  The Financial Aid Committee will review and provide a response as soon as possible.
